Since the reform of the GOT (Gebührenordnung für Tierärzte, the official German fee scale for vets), vet bills have become significantly more expensive – a major operation quickly reaches four figures. Surgery cover (OP-Schutz) covers exactly this risk at low cost, while comprehensive health cover (Krankenvollschutz) also covers outpatient treatment and medication.

Important for context: all premiums come from the FinanceAds comparison calculator, updated daily, and are calculated for a 2-year-old dog without excess (breed group 1). For older dogs, other breeds or with an excess, the premium will differ – the insurer's offer is always what counts.

Surgery cover or comprehensive cover?

For dogs there are two types of product. Surgery cover (OP-Schutz) pays only for operations – precisely the bills that quickly run into four figures (cruciate ligament rupture, foreign body surgery, tumour removal). It costs only a fraction of comprehensive cover. Comprehensive health insurance (Krankenvollversicherung) also covers outpatient treatment, medication and diagnostics; depending on the policy, an annual reimbursement limit applies. If you are looking for basic protection against major risks, surgery cover is cheaper; if you also want to cover the many small bills, you need comprehensive cover.

What to look out for when comparing

  • GOT reimbursement rate – the official fee scale for vets (GOT) normally allows 1 to 3 times the standard rate, and at least 2 and up to 4 times the rate for emergency services. Good policies reimburse up to 4 times the rate, weaker ones cap earlier.
  • Annual maximum – crucial for comprehensive policies: is reimbursement unlimited or capped at a few hundred euros?
  • Excess (Selbstbeteiligung) – lowers the premium but costs you money with every claim. The premiums shown here are calculated without an excess.
  • Waiting period – up to three months without benefits is common. Some providers waive it.
  • Age & breed – the premium rises considerably with the dog's age. Taking out a policy early is almost always cheaper.
  • Pre-existing conditions – conditions that are already known are usually excluded.

How much does a vet bill cost?

Since the reform of the GOT (Gebührenordnung für Tierärzte, the fee scale for vets), treatment costs have risen noticeably. Routine procedures cost in the mid three-figure range, larger operations such as a cruciate ligament rupture or spinal disc surgery quickly reach several thousand euros – with surcharges for emergency services on top. This is exactly the risk pet health insurance cushions. Important: the insurance does not replace pet owner's liability insurance (Tierhalterhaftpflicht) – that pays for damage your dog causes to others.

How a pet health insurance claim works

As a rule you pay the vet's bill up front: you settle the treatment at the practice and then submit the invoice to the insurer, usually digitally via an app or upload. The insurer checks whether the treatment is covered and transfers the reimbursement minus any agreed excess. It is important that the invoice is issued under the German veterinary fee schedule (GOT) and lists diagnosis and treatment in a comprehensible way.

The GOT sets a basic rate for veterinary services, which the practice may increase up to three times depending on difficulty, time and circumstances. In emergency out-of-hours service at least double and at most four times the basic rate is customary. Whether the insurer reimburses these rates in full depends on the plan. Weaker plans cap at around double the rate, so you pay part of emergency treatment yourself.

Many plans also have an annual reimbursement limit. With surgery cover it is often higher, because only rare but expensive procedures are insured. With comprehensive cover and a reimbursement limit, repeated treatments of a chronically ill animal can use up the limit quickly. Check in the plan whether the cap applies per insurance year or per illness.

Worked example: an operation with and without insurance

A worked example using freely chosen sample values that does not reflect the terms of any specific plan: a dog suffers a cruciate ligament rupture and the bill for surgery, anaesthesia, aftercare and medication comes to €2,400. With surgery cover that has a €100 excess and full reimbursement up to three times the GOT rate, the insurer reimburses €2,300. You pay only the €100 yourself.

Without insurance you would have to find the entire €2,400 from your own funds. If the premium in the example is €25 a month, you pay €300 a year. After eight years you would therefore have paid €2,400 in premiums, exactly as much as this one operation. If the loss occurs earlier or several treatments are needed, the insurance works out cheaper; if the dog stays healthy, you have paid premiums without receiving a benefit.

This calculation shows that pet health insurance is mainly protection against peak risks. If you would rather save the premiums yourself, check realistically whether you could find several thousand euros at short notice in an emergency. Anyone who cannot or does not want to do that often opts for insurance, even though it does not pay off every year.

Age, premium and cancellation

With most plans the premium rises with the dog's age, because illness costs increase over a lifetime. Differences by breed group, size and health risks also play a role. Some insurers no longer accept dogs above a certain age or exclude certain breeds. Taking out cover early usually secures you a lower starting premium, although this can later be adjusted because of age.

When you sign up you must answer questions about health and previous treatment truthfully. Under section 19 of the Insurance Contract Act (VVG) the insurer may withdraw from the contract or refuse payment if you give false information. Illnesses already known and many hereditary conditions are generally excluded or covered only after a waiting period stated in the terms.

Contracts often run for one year and renew automatically, with a fixed term of three years at most (section 11 VVG). If the insurer raises the premium without improving cover, you have a special right of cancellation under section 40 VVG, which you must use within one month. After a claim, either side may also cancel under section 92 VVG. Compare not just the starting premium but also the rules on premium adjustments.

Which cover suits whom

Surgery cover suits owners who want to insure against rare but high bills and can pay smaller costs for vaccinations, routine check-ups and mild illnesses themselves. It is cheaper and its premium usually stays more stable. Comprehensive cover is worthwhile above all for owners who want to spread predictable costs evenly or who have a dog with an increased illness risk.

When comparing, work out the benefits in concrete terms. Check whether dental treatment, physiotherapy, neutering and preventive care are included and how large the annual budget for them is. Also see whether the plan reimburses treatment abroad within the EU and whether the waiting period is waived for accidents. A low premium is of little use if the reimbursement limit is reached with the first major treatment.

Pet health insurance does not replace liability cover. Under section 833 of the German Civil Code (BGB) the keeper of an animal is liable for damage the animal causes to others, which can become very expensive. Pet owner liability insurance protects against this. In some federal states it is even compulsory for dogs, so you should ask your local public order office.

Frequently asked questions

How much does dog health insurance cost?
Pure surgery cover is considerably cheaper than comprehensive health cover. How high your premium is depends mainly on the age and breed of the dog, the scope of cover and any excess. The premiums in the table apply to a two-year-old dog without an excess.
Is pet health insurance worthwhile?
It is worthwhile above all as protection against individual large bills. An operation can cost several thousand euros – if you cannot or do not want to raise this amount at short notice, you are well advised to take out at least surgery cover.
Is there a waiting period?
Usually yes: no benefits are paid in the first weeks to months after the policy starts. For accidents, the waiting period often does not apply. Some providers waive it completely – these policies are marked accordingly in the table.
What does 'reimbursement up to 4 times the GOT rate' mean?
Vets bill according to the official fee scale for vets (GOT) and may multiply the standard rate depending on the effort involved. If a policy reimburses up to 4 times the rate, you are far less likely to be left with costs even for complex treatment and emergency services.
Are neutering and preventive care covered?
Not in every policy. Some comprehensive policies contribute to castration, sterilisation, vaccinations and other preventive care – often with an annual budget. The table shows which policies include these benefits.
Are pre-existing conditions covered?
As a rule, no. Insurers usually exclude conditions that are already known when the policy is taken out. That is why it is worth taking out a policy while the dog is young and healthy.
Do I have to pay the vet's bill up front?
As a rule yes. You pay for the treatment at the practice and submit the invoice to the insurer, which transfers the reimbursement to you. Some providers allow direct settlement with the practice, but this is not common.
Can I cancel the insurance if the premium goes up?
Yes. If the insurer raises the premium without improving cover, you have a special right of cancellation under section 40 VVG. You must use it within one month of receiving the notice.
How does the dog's age affect the premium?
As the dog gets older the premium usually rises, because illnesses become more frequent. Signing up young is therefore almost always cheaper. Some insurers no longer accept older animals.
Does health insurance replace dog liability insurance?
No. Health insurance pays for treatment of your own dog. Damage the dog causes to others is covered only by pet owner liability insurance.
