Change in the law under way: the amendment to the Medical Cannabis Act (MedCanG)

The draft bill provides that a first prescription would only be permitted after in-person doctor-patient contact – an online questionnaire or video consultation would then no longer be sufficient. A ban on mail-order sales of cannabis flowers is also planned (delivery by the pharmacy's own courier would remain allowed), and repeat prescriptions would need an in-person appointment at least once every four quarters. As of 30 October 2026 the law (Bundestag printed paper 21/3061, hearing on 14 January 2026) has not been passed; it is with the Bundestag's Health Committee. The offers and prices given here therefore still apply but may change at short notice.

What a cannabis prescription really costs

The prescription fee is the smallest item. Anyone requesting a cannabis prescription online ends up paying three things: the medical service (9.90 € to 53.54 €), the medication itself and usually shipping. With a typical monthly quantity of 30 grams at 8 €, the flowers come to 240 € – next to that, a 5 € difference in the prescription fee hardly matters.

That is why the number of connected pharmacies is the more important metric. The price per gram is set by the pharmacy, not the prescription platform. If, as with DoktorABC, you can choose from 700 pharmacies, you are more likely to find your strain and can compare prices. With CanDoc and its three partner pharmacies you pay whatever is on their shelf.

Watch out for the repeat prescription

Cannabis therapy is rarely a one-off. This is exactly where the field splits: GreenMedical and Dr. Ansay charge the same price for the repeat prescription as for the first. Bloomwell charges 28.25 € per repeat prescription without an appointment – over twelve months that is around 339 € in doctor's fees alone, compared with about 119 € at GreenMedical. DoktorABC states no price for repeat prescriptions at all.

Money back on rejection

Not every request leads to a prescription. Three providers explicitly guarantee this: Dr. Ansay refunds the full amount, GreenMedical advertises “no cost risk on rejection”, and Privatrezept.net automatically refunds the deposit. Bloomwell, by contrast, explicitly excludes a refund for the free prescription request. DoktorABC, CannaZen and Nordleaf say nothing on their sites – if in doubt, ask beforehand.

Questionnaire or doctor consultation?

Seven of the eight providers use an online questionnaire reviewed by a doctor. Only Bloomwell requires a genuine initial consultation – which costs more. The planned amendment would, however, require in-person contact; a video call would then no longer be enough for a first prescription, only the practice appointment (71.08 € at Bloomwell). If you are planning long-term therapy, factor that in.

Frequently asked questions about cannabis prescription costs

How much does an online cannabis prescription cost?

The prescription fee alone is normally between 9.90 € and 25.22 € at the providers we checked. Nordleaf, GreenMedical and CannaZen are cheap at around 10 €; at the top end is Bloomwell with 25.22 € for the prescription request and 53.54 € for an online initial consultation. CanDoc also advertises a promotion “from 0 €” but does not state its regular price. Note: this covers only the medical service. The flowers themselves cost roughly 4 to 9 € per gram depending on the strain and come on top, as does shipping in many cases.

Do I get my money back if no prescription is issued?

Not everywhere. Refunds are explicitly promised by GreenMedical, Dr. Ansay, Privatrezept.net. Dr. Ansay refunds the full amount, GreenMedical advertises “no cost risk on rejection”, and Privatrezept.net automatically refunds the deposit in full. Bloomwell, by contrast, explicitly excludes a refund for the free prescription request. DoktorABC, CannaZen and Nordleaf say nothing about it on their websites.

How many pharmacies are connected – and why does it matter?

The range is huge: CanDoc works with three partner pharmacies, while GreenMedical lists 571 pharmacies by name with their address. The larger the network, the more likely your preferred strain is available and the easier it is to compare the price per gram – because the pharmacy sets that, not the prescription platform. With only three pharmacies you effectively pay whatever is on their shelf.

Does health insurance pay for the prescription?

Not with any of the telemedicine providers compared here. These platforms work exclusively on private prescriptions, so you pay yourself. Statutory health insurance (GKV) can only cover costs via a regular doctor's practice, requires an application before treatment starts, and in practice only applies to serious conditions when other therapies have been exhausted. Bloomwell points out that follow-up services may be reimbursable by private insurers.

Is a questionnaire enough or do I need a video appointment?

Seven of the eight providers use an online questionnaire only, which a doctor reviews. Only Bloomwell relies on a genuine initial consultation by video or in a practice – which also explains the higher price. Privatrezept.net offers an optional video call for 29.99 €. GreenMedical reserves the right in its small print to require a video consultation “if necessary”.

Will the planned amendment to the Medical Cannabis Act (MedCanG) change prices?

Possibly fundamentally. The draft bill provides that a first prescription would only be permitted after in-person doctor-patient contact – a questionnaire or video consultation would then no longer be enough. That would make the business model of the pure online-questionnaire providers obsolete, and prices would probably move towards the level of a practice appointment. For repeat prescriptions the draft requires an in-person appointment at least once every four quarters, with telemedicine remaining permitted in between; shipping of cannabis flowers is also to be banned (delivery by the pharmacy's own courier would remain possible). The government bill (Bundestag printed paper 21/3061) had its first reading on 18 December 2025, and a public hearing was held in the Health Committee on 14 January 2026 – as of the end of October 2026 it has not been passed. The terms given here currently apply.
